Carers Trust Rapid Responder
Carers Trust Cambridgeshire would like to recruit a Rapid Responder to provide support (sometimes at short notice) with hospital discharge and/or care to relieve stress on the family carer of the person with care needs. To be able to assess a family situation, including care needs, to ensure a safe and effective response to their needs.
